Personal Data

When we refer to “Personal Data” we mean any information relating to an identified natural person who can be directly or indirectly identified by a name, identification number, location data, or an online identifier.

MyMemoryMentor.com, directly and through third-parties, collects information that you provide to us from your use of the site. The information we collect and how we use it depends on how you use and access the Services provided on the site. We collect information, including Personal Data, as follows:

  • Your Form Submissions on the Site—My Memory Mentor collects your Personal Data when you fill in any form on the site that may include signing up for an online assessment, training resource, newsletter, giveaway, free downloadable resources, or contact us forms. We use this information to respond to your request. We process this Personal Data as long as we have legitimate interest or based on your consent. You can have your information removed by unsubscribing from an email using the unsubscribe link or by emailing [email protected].
  • Your Login and Profile Submission—My Memory Mentor allows users to review assessment and learning content as well as provide feedback or comments. This can be published on the site along with profile information that includes your supplied first and last name. Users must establish a user ID and password to login to the User Dashboard sites. To establish a User ID and password, we require your first name, last name, e-mail address, organization name, country location, zip code. This information is stored on My Memory Mentor secure servers and is not shared with any outside parties. We use this information to verify your identity as a valid user and to allow you to post comments and perform other actions on our website. You can have your information removed by sending a request to [email protected].
  • Your Location—My Memory Mentor collects location data from personal data you submit to us or from using the third-party tool, Google Analytics that reports location based on your device’s IP address. We use this information to process your requests for materials that you may request get mailed to you or to inform future development of location specific content.
  • Contests, Sweepstakes, and Promotions—My Memory Mentor can provide its users with the ability to participate in contests, giveaways, sweepstakes, and other similar promotions that we or third parties may sponsor. Participation may require sharing your personal data, and you agree that your data will be shared with the partner. We/they use the information to provide you information about the contest and occasionally further marketing materials. As a condition to receiving any awards or prizes, you may be required to provide additional information, to sign a release, or to authorize the use of information about you in marketing materials. Sensitive Personal Data 

My Memory Mentor may collect sensitive personally identifiable information (“PII”) associated with certain customers or customers’ clients, such as individuals’ Social Security Numbers and dates of birth, in order to provide certain product and service offerings.  We will only collect this PII if it is necessary to fulfill certain product orders and only when voluntarily provided by the customer.  We will only use it for the limited, permissible purpose for which it is collected and we will take commercially reasonable steps to adequately secure the information. We may also share this PII with trusted third parties for the limited, permissible purpose of fulfilling the associated product orders. Additionally, sometimes data subjects will send us sensitive personally identifiable information without our asking. When we become aware of this, we will delete the sensitive personally identifiable information.

Usage Data

We have designed mymemorymentor.com  so that visitors may anonymously access our content without revealing their identities. You can browse the Site without telling us who you are or revealing any personal data about yourself.

Advertisement

This “Usage Data” can include demographic information, interests, search queries, and online activity. Automatic Usage Data Collected through Cookies or Similar Technologies—with your consent or as otherwise permitted by law, when you visit mymemorymentor.com including our social media pages or open our emails, we and other third-party vendors, such as advertising networks, social media widgets, affiliate partners, and analytics providers, collect certain information by automated means such as cookies, web beacons, and web server logs. What are Cookies, Web Beacons, Pixel Tags and GIFs and how are they used?

First party cookies are sent by, or on behalf of, the company whose website you visit. Third party cookies are sent by any other company. My Memory Mentor uses first and third-party cookies.

Third Party Marketing

The contact information of individuals who have signed up to participate in one of our programs will be shared with the third-party sponsor of the program. Note that if your professional contact information is in a different third party’s marketing directory, you will need to request removal with such third party directly.

How We Protect Personal Data

Mymemorymentor.com has commercially reasonable safeguards in place to protect against unauthorized access to, use, or disclosure of Personal Data we maintain. My Memory Mentor manages a strict due diligence program for all our vendors to employ adequate data collection, processing, transfer, management, and security measures in carrying out their services on our behalf. Access to personally identifiable information is authorized only for those who have a business need for such access such as our webmaster to help reset your password or delete your account upon your request and our communications manager who is responsible for sending you email communications based on your opt-in preferences. We only keep the information we collect for as long as the purpose we collected it for, and we delete it upon request. Data Retention

My Memory Mentor will retain your information for as long as your account is active or as needed to provide you services. If you wish to cancel your account or request that we no longer use your information to provide you services, contact My Memory Mentor at [email protected]. We will retain and use your information as necessary to comply with our legal obligations, resolve disputes, and enforce our agreements.

Sale and Transfer of My Memory Mentor

If My Memory Mentor is sold, the information collected will be considered an asset that can be transferred.

Children under Age 13

My Memory Mentor does not knowingly collect or solicit personal information directly from anyone under the age of 13 or knowingly allow such persons to register. If you are under 13, please do not attempt to register directly for My Memory Mentor or send any information about yourself to us, including your name, address, phone number, or email address. Everyone under the age of 13 must let their parent/legal guardian or authorized person (e.g. school teacher or counselor) handle sharing of any personal information to or on My Memory Mentor or use the Site in any way. In certain situations, My Memory Mentor may be required to disclose personal information in response to lawful requests by public authorities, including to meet national security or law enforcement requirements.

General Data Protection Regulation (GDPR)

We recognize that some of the information we collect may be classified as “personal data” under European Union (EU) law as it is information relating to an individual (e.g. a sole trader, a partnership, a company director, a beneficial owner, a trustee, a professional contact, etc.) residing in the European Union. Different parts of the GDPR apply when we act as a processor and we take these obligations very seriously. The above notice does not apply to our data received from our customers as this does not become My Memory Mentor data (unless the customer has expressly agreed to this). We handle the data our customers provide us in strict accordance with the agreement – and only for the purposes of this agreement.

This dispute resolution program covers both offline data collection (e.g. personal information collected through our call centers or via postal mail) and online data collection (e.g., personal information collected via the Internet).
